This privacy policy (hereinafter the “Policy”) sets out how Clinique médicale RocklandMD Inc. collects, uses, discloses, retains, and destroys your Personal information. For ease of reading, we will use the terms “we”, “us” or “the clinic” interchangeably throughout the Policy.
We may provide you with additional information regarding the handling of your Personal information at the time of collection. We may also obtain your specific consent in certain cases regarding the collection, use, disclosure, and retention of your Personal information.
The Policy applies to Personal information collected by us, regardless of how it is collected, whether in person, by telephone, by email, or through our website.

1. Training
We train and raise awareness among our staff and professionals through the following activities:
- Annual reminder of confidentiality obligations; and
- Vigilance training about Cybersecurity and protection of personal information.
2. Important Definitions
In this Policy, the following terms shall have the meanings ascribed to them:
- “Privacy Incident” means (i) unauthorized access to personal information by law; (ii) unauthorized use of personal information by law; (iii) unauthorized disclosure of personal information by law; or (iv) loss of personal information or any other breach of the protection of such information.

3. How we will handle your Personal Information
Collection of Personal Information
In the course of its activities, Clinique Médicale RocklandMD collects various types of Personal Information. Consent for the collection of this data is generally obtained prior to collection, either implicitly or explicitly. Personal Information is typically collected directly from the individual requesting the service. In exceptional circumstances, Personal Information may be received from an authorized third party. The Personal Information we collect includes, but is not limited to:

- Email address; First and last names of parents;
- Demographic data (gender, date of birth);
- Billing information;
- Health insurance number;
- Medical records and notes;
- Results of medical examinations.
At the time of collecting Personal Information, staff at RocklandMD Medical Clinic inform individuals of the following:

- The methods by which the Personal Information is being collected;
- The individuals or entities who will have access to the Personal Information;

4. Use of Personal Information
- The Clinic may use Personal Information for the following purposes:
- To provide and maintain our medical services, including conducting follow-ups.
- To communicate with you via email, telephone, SMS, or other equivalent electronic means.
- To provide you with news, special offers, and general information about other goods, services, and events that we offer, unless you have opted out of receiving such information.
5. Access to Personal Information
Personal Information is securely stored using appropriate software specific to the type of Personal Information. The various software applications used, and their purposes are described in the section below.
Access to Personal Information within the different software applications is restricted to users who require such access in the performance of their duties. Each access is protected by a unique password. All accesses are timely revoked when the individual is no longer an employee or self-employed worker of Clinique Médicale RocklandMD. Access management to the different software applications is overseen by the IT team.
We may share your Personal Information in the following situations:
- With service providers: We may share your Personal Information with service providers to deliver the services you wish to obtain. The different providers and their security measures are detailed in section 6 below.
- With affiliated companies: We may share your Personal Information with our affiliated companies, in which case we will require these affiliated companies to abide by this Privacy Policy. Affiliated companies include our parent company and any subsidiaries, or other companies under common control.
- With courts, authorities, and other organizations: In certain situations, we may need to share Personal Information about you with courts, law enforcement authorities, and other organizations. This sharing may be mandatory to comply with requests, warrants, orders, or to fulfill our insurance obligations.
Personal Information related to medical records is retained for as long as necessary according to the regulations of provincial medical regulatory bodies.
All other Personal Information is retained for as long as necessary in compliance with applicable laws.
When the retention of such Personal Information is no longer necessary, Clinique Médicale RocklandMD Inc. ensures it is securely destroyed.

7. Security Measures and Inherent Risks
We adhere to industry standards for security in our field of activities to protect the information we collect and receive. Specifically, we have implemented appropriate physical, technical, IT, and administrative protection measures to safeguard your Personal Information against privacy incidents. All employees and service providers of Clinique Médicale RocklandMD have signed confidentiality agreements and committed to adhering to the strictest policies regarding personal information security.
Despite these measures, due to the inherent risks associated with the use of computer systems, we cannot guarantee the security and confidentiality of the information you transmit or provide to us, and you do so at your own risk. If you have reason to believe that personal information has been compromised, please contact us using the contact information provided above.
8. Complaints Management
Any complaint regarding the management of Personal Information by the clinic must be made in writing and addressed to the Privacy Officer by email at confidentialite@rocklandmd.com or by mail to the address provided above.
All complaints are handled confidentially. Only the information necessary for processing the complaint will be shared with the personnel of RocklandMD Medical Clinic who need to be aware of it in the performance of their duties.
The processing time is 45 days from the date of receipt of the complaint or inquiry.
9. Application
This Policy applies to executives, subcontractors of the Clinic.
This Policy is implemented for all Personal Information and confidential data collected, managed, and used by the clinic.
